Please download the Application Form to your computer.

• For best results, please download and use Adobe Acrobat Reader DC (https://acrobat.adobe.com/us/en/acrobat/pdf-reader.html) to complete the form.

Please make a scanned copy of your identification (e.g., Driver’s License or Passport) that shows your photo and date of birth. The identification will be used to verify your age for the piano competition. 

• Please redact (i.e. blackout) any other unwanted information to be shown in this document for security purposes. The Nashville International Chopin Piano Competition will not be held responsible in the event of a data security breach.

Please email the completed Application Form and scanned copy of your identification to admin@nashvillechopin.org which includes the following: 

2 letters of recommendation from music professionals.

High-quality professional photograph. 

YouTube link of audition recording (live or studio performance within last 12 months).

• Ages 16 - 22: 3 works​ by Frédéric Chopin

• Ages 11 - 15: 2 works by Frédéric Chopin

• Ages 6 - 10: 1 work by Frédéric Chopin

Please pay the $250 USD application and $35 USD processing fee to complete your application.​
